HUMAN error was to blame for the tagging failure that allowed a teenager to leave his home and murder a neighbour, it was revealed yesterday.

The Scottish Executive said the tag was wrongly set to go off at "medium" range, enabling Callum Evans, 18, to leave the flat in Glasgow undetected. The tag should have been set at "short".
