THERE are few things people look forward to less than going to the dentist: the awkward flick through old magazines in the waiting room; the inevitable calling of your name to announce the dentist is ready to see you; the mechanical drone as the chair reclines; the metal probes, the injections, the small cup of pink water. And, if you're really in luck, the drill.

But it is not the pain of the treatment that I dread the most. It is the pain of paying the bill.
